İDRAR KÜLTÜRÜ SONUÇLARININ KATETER İLİŞKİLİ ÜRİNER SİSTEM ENFEKSİYON TANISINDA KLİNİK ANLAMIThe Clinic Significance Of Urinary Culture Results For Catheter-Related Urinary Tract Infection

ÖZETAmaç: Yoğun Bakım Üniteleri (YBÜ), hayati fonksiyonlarındaki risk nedeniyle özel bakım gerektiren hastalarıntedavi edildikleri birimlerdir. Bu hastalarda invaziv uygulamaların yaygın kullanılması başta olmak üzerebirçok faktöre bağlı olarak gelişen hasta bakımı ile ilgili enfeksiyonlar (HBİE) sıklıkla görülmektedir. Bu çalışma,YBÜ’de kateter ilişkili üriner sistem enfeksiyonu (KİÜSE) tanısı konan hastaların kültür sonuçlarının kliniktanı ile korelasyonunun değerlendirilmesi amacıyla yapılmıştır.Gereç ve Yöntemler: Ocak 2018-Ocak 2019 tarihleri arasında hastanemiz YBÜ’lerinden gönderilen 2290hastaya ait 4281 idrar kültürü sonucu ve aktif sürveyans sonuçları değerlendirilmiştir.Bulgular: Hastaların 285’inde (%12,5) anlamlı kültür pozitifliği saptanmıştır. Üreme olan örneklerin 130’unda(%45,6) Candida spp., 155’inde ise (%54,4) bakteriyel patojenler saptanmıştır. En sık izole edilen bakterilerEnterococcus spp. (%24,9) ve E. coli (% 16,8) olarak bulunmuştur. Aynı zaman periyodunda aktif sürveyansile hastaların 11’inde (%0,48) KİÜSE tanısı konmuştur. Bu hastaların 5’inde (%45,5) Enterococcus spp. ve3’ünde (%27,3) E. coli en sık raporlanan mikroorganizmalar olmuştur.Sonuç: Aynı hasta grubunda kültür sonuçlarına dayalı ve klinik değerlendirme kriterlerine dayalı idrar yoluenfeksiyonu tanısı arasında yüksek oranda fark tespit edilmiş olup, bu konunun irdelenmesinin hastane sürveyansıaçısından daha faydalı olacağı sonucuna varılmıştır.Anahtar Kelimeler: Yoğun bakım ünitesi; kateter; üriner sistem enfeksiyonu; aktif sürveyansABSTRACTObjective: Intensive Care Units (ICUs) are the units in which patients requiring special care are treated dueto the risk of their vital functions. In these patients, patient care-related infections (PCRI) are frequentlyseen due to many factors, especially the widespread use of invasive procedures. This study was conductedto evaluate the correlation of culture results and clinical diagnosis of patients with catheter-related urinarytract infection (CRUTI) in the ICU.Material and Methods: 4281 urine culture results of 2290 patients and active surveillance results sent fromICUs of our hospital between January 2018 and January 2019 were evaluated.Results: Culture positivity was found in 285 (12.5%) patients. Candida spp., was found in 130 (45.6%) anda bacterial pathogens in 155 (54.4%) culture positive samples. The most frequently isolated bacteria wereEnterococcus spp., (24.9%) and E. coli (16.8%). In the same time period, 11 (0.48%) of the patients were diagnosedwith CRUTI by active surveillance. In 5 (45.5%) of these patients Enterococcus spp. and in 3 (27.3%)of them E. coli were the most commonly reported microorganisms.Conclusion: In the same patient group, a significant difference was found between the diagnosis of urinarytract infection based on culture results and clinical evaluation criteria, and it was concluded that the examinationof this issue would be more beneficial for hospital surveillance.Keywords: İntensive care unit; catheter; urinary tract infection; active surveillance
