ENDOMETRİAL ÖRNEKLEME SONUÇLARIMIZ: 365 OLGUNUN ANALİZİ Results of Our Endometrial Samplings: Analysis of 365 Cases Anıl TURHAN ÇAKIR, İsa Şükrü ÖZ, Burak ÜN ÖZET Amaç: Çalışmamızın amacı çeşitli nedenler ile yapılan endometrial örnekleme işleminin; endikasyonları ve histopatolojik tanılarının değerlendirilmesi. Gereç ve Yöntem: Bu çalışmada Ocak 2015-Mart 2018 tarihleri arasında kliniğimizde çeşitli nedenler ile endometrial biyopsi yapılan 365 hastanın endometrial biyopsi sonuçları retrospektif olarak değerlendirildi. Bulgular: Hastaların yaş ortalaması 46,7±9,9 idi. Endometrial biyopsi endikasyonları sırasıyla anormal uterin kanama (%74), postmenopozal kanama (%11,5) ve postmenopozal endometrial kalınlık artışı (%14,5) idi. Histopatolojik sonuçlar ise en sık proliferatif endometrium (%32,1) sonrasında sırasıyla sekretuar endometrium (%23), endometrial polip (%16,4), yetersiz materyal (%12,9), atrofi (%8,8), basit atipisiz endometrial hiperplazi (%5,2), endometrial kanser (%1,1) olarak raporlandı. Premenopozal dönemde sekretuar endometrium ve proliferatif endometrium yüksek oranda bulunurken (p<0.001); postmenopozal dönemde ise atrofi (p<0.001), yetersiz materyal (p<0.001) ve endometrium kanseri (p=0.004) yüksek oranda bulundu. Sonuç: Endometrial biyopside esas amaç endometrium kanserini saptamaktır. Özellikle postmenopozal dönemde endometrium kanseri açısından dikkatli olunmalıdır. Anahtar kelimeler: Uterus kanaması; Menoraji; Metroraji; Biyopsi; Histopatoloji ABSTRACT Aim: Aim of the recent study to evaulate the relation between indications and histopathological diagnosis of the endometrial sampling procedures performed for various indications. Material and Methods: In this retrospective study endometrial biopsies of 365 patients who had undergone endometrial biopsy due to various causes between January 2015 and March 2018 at our clinic were evaluated retrospectively. Results: The mean age of patients was 46,7±9,9. The indications were abnormal uterine bleeding (%74), postmenopausal bleeding (%11.5), postmenopausal increased endometrial thickness (%14,5). The most common histopathological result was proliferative endometrium (%32.1) the others respectively; secretory endometrium (%23), endometrial polpys (%16.4), insufficient material (%12.9), atrophy (%8.8), simple endometrial hyperplasia without atypia (%5.2), endometrial cancer (%1.1). In premenapousal group had a higher incidence of secretory and proliferative endometrium (p<0.001) while in postmenapousal group had a higher incidence of atrophy, insufficient material and malignancy (p<0.001). Conclusion: In endometrial biopsy the main aim is to determine endometrial cancer. Particular attention should be paid to endometrial cancer in postmenopausal period.
