CHARCOT-MARIE-TOOTH TANILI HASTADA SAKRALGLUTEAL CERRAHİDE REGİONAL ANESTEZİ İLE

Anaesthetic Management with Regional Anaesthesia in a
Charcot-Marie-Tooth Diagnosed Patient who is Under Sacral-
Gluteal Surgery in

ÖZET
CMTH herediterdir, motor ve duyu kaybına neden olan bir nöropati olarak bilinir. Nöropatik bir
rahatsızlık olması, postür bozuklukları ile seyretmesi, malign hipertermi ve myopati riski nedeniyle
anestezi yönetimi sıkıntılar içermektedir.
19 yaşında erkek hasta pilonidal sinüs nedeniyle operasyona alındı. Hastaya operasyon için rejyonel
anestezi uygulanması planlandı ve spinal anestezi işlemi uygulandı. Hasta saddle blok uygulamak
için 5 dk süre ile oturtuldu. Hastanın operasyon sırasında herhangi bir ağrı şikâyeti olmadı.
Operasyon süresince tansiyon arterial, nabız, oksijen satürasyonu değerleri normal aralıkta
seyretti. Post operatif 2. saatte kalça ve bacaklarında sıcak-soğuk diskriminasyonu mevcuttu ve
pin-prick testi pozitif idi. Hastanın servis takiplerinde herhangi bir sıkıntı görülmedi. Hasta operasyonun
48. saatinde eksterne edildi. Rejyonel anestezi nöropati riskini artırsa bile CMTH’de
nispeten daha güvenli olduğu tarafımızca düşünülmektedir.

ABSTRACT
CMTD is hereditary, known as neuropathy that causes motor and sensorial deficiency. In consequence
of its being a neurophatic disease, related with posture disorders and malign hyperthermia;
anaesthesia managment of patients with CMTD includes hazards.
19 years old male patient was taken an operation for pylonidal sinus disease. Regional anaesthesia
with spinal anaesthesia was planned and administered for the patient. Subsequent to the
spinal injection, seating position was given to the patient for 5 minutes to perform saddle block.
Patient didn’t suffer from any pain in the operation room. Arterial pressure, cardiac pulse rate
and oxygen saturation values was in normal range. There was cold and hot discrimination on leg
and hip at postoperative 2nd hours and pi-prict test was positive. He hadn’t a problem in surgery
clinics visits. He was discharged at 48th hours of hospitalization. Even if regional anaesthesia is
believed to increase the risk of neuropathy, our decision is it’s more confident from other anaesthesia
methods in CMTD.
